• Enter your Full Name in BLOCK LETTERS (as per official records).
• Fill in your Father’s & Mother’s Name correctly.
• Provide your Date of Birth (DD/MM/YYYY).
• 📞 Step 2: Contact Information
• Add your Mobile Number (Primary & Alternate).
• Provide a valid Email ID for communication.
• 🏡 Step 3: Address Details
• Mention your Permanent Address with Pin Code.
• If different, provide your Current Address separately.
• 🎓 Step 4: Educational Qualifications
• List your previous academic records (School/College, Year, Percentage).
• Attach copies of mark sheets & certificates as required.
• 🎯 Step 5: Course Selection
• Choose your desired course/program from the options available.
• If applicable, mention your preferred batch/timing.
• 🆔 Step 6: Identification & Documents
• ️ Attach a recent passport-size photograph.
• Provide a photocopy of a government-issued ID (Aadhaar, Passport, Voter ID).
• 💰 Step 7: Payment Details (If Applicable)
• If paying online, enter the transaction/reference number.
• If paying in cash, mention the amount & date.
• ✅ Step 8: Declaration & Submission
• ️ Carefully read the declaration section before signing.
• Sign the form and get a parent/guardian’s signature (if required).
• Submit the form at the office (or via the instructed mode).
